Minimum Premiums:

• Package Policy without Professional E&O - $500
• Package Policy with Professional E&O - $1,000
• Umbrella Policy not including the Professional E&O - $1,000
• Umbrella Policy including the Professional E&O - $1,500
• Property Policy - $150 Crime Policy - $1,000

Submission Requirements:

• A complete submission is requested, along with thefollowing, to assure best terms & prices are offered:
• Fully completed & signed All Risks application
• Fully completed & signed Acord forms
• Five years currently valued loss runs
• Website address of the insured
